Make an appointment:

To learn more about working with me, please call (312) 772-3038, or text or email me (kelinlcsw@gmail.com). We will schedule a free, 20-minute phone consultation to assess fit and answer your questions. I will then send you a link to complete intake forms and review access to the video platforms for teletherapy.

Insurance and Payment:

I currently in network with BCBS PPO and BlueChoice PPO.

You may also elect to pay out of pocket. My full rate is $170/session. If you have another insurance, you may check your out of network benefits and submit for reimbursement. I offer reduced rates (down to $20) to a limited number of uninsured clients with significant structural barriers to mental health care. I value providing accessible, affordable therapy.

I electronically process credit/debit/or HSA/FSA card payments.

Office and Accessibility:

The entrance/waiting area

My office is accessible via the brown line, Ashland bus, and Irving Park bus. There are six stairs up to the building, and an elevator to the 2nd floor. Unfortunately there are not gender neutral bathrooms at this time, but we are working with the building.

The office where we meet, chairs are >6 feet apart.

No Surprises Act - Notice to clients and prospective clients:

Under the law, health care providers need to give clients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services, including psychotherapy services.

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including psychotherapy services.

You can ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service, or at any time during treatment.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.

For questions or more information about your right to a Good Faith Estimate, or how to dispute a bill, see your Estimate, or visit www.cms.gov/nosurprises
